Looking for a holistic and flexible healthcare option for your children? We offer a Family Membership to provide an affordable integrative approach for families with children 18 and under.

Our Family Functional Medicine Membership provides affordable, comprehensive care tailored for families with children under 18. A membership provides integrative visits that can be used for any child in the family for a set monthly price. 

What’s Included: 

• In-depth initial consultation 

•      Follow-up visits, including: 
•      Well-child checkups 
•      Acute/sick visits 
•      Chronic condition management
•      Sports physicals 

• Visit sharing: Use visits as needed across all children in your immediate family under  age 18.

• All visits can be used anytime within 2 years of enrollment

Membership Options:

•  6-Visit Family Package 
•  • 1 initial visit + 5 follow-ups 
• • Payment plan: $305 upfront, then $97/month for 12 months 

• 10-Visit Family Package 
• • 1 initial visit + 9 follow-ups • 
• Payment plan: $305 upfront, then $175/month for 12 months 

• 16- Visit Family Package
• • 1 initial visit + 15 follow-ups 
• Payment plan: $305 upfront, then $290/month for 12 months 

Need More Visits? 

• Additional visits available (cost varies by time and acuity) 

• Option to upgrade to a larger package anytime 

· Insurance will be bill after each appointment for eligible services including lab work.

 Any reimbursement comes directly to you.  

· All visits can be used anytime within 2 years of enrollment

· If you cancel early, only pay for the visits you’ve used.

· Membership cover appointments and provider time- supplements and external medical services are not included.
