Our Family Functional Medicine Membership provides affordable, comprehensive care tailored for families with children under 18. A membership provides integrative visits that can be used for any child in the family for a set monthly price. 

What’s Included: 

• In-depth initial consultation 

• Follow-up visits, including: 

- Well-child checkups 

- Acute/sick visits 

- Chronic condition management

- Sports physicals 

• Visit sharing: Use visits as needed across all children in your immediate family 

• Care coordination & secure messaging with your provider 

Membership Options: 6-Visit Family Package 

 • 1 initial visit + 5 follow-ups 

• Payment plan: $305 upfront, then $95/month for 12 months 

10-Visit Family Package –

• 1 initial visit + 9 follow-ups • 

Payment plan: $305 upfront, then $170/month for 12 months 

Need More Visits? 

• Additional visits available (cost varies by time and acuity) 

• Option to upgrade to a larger package anytime 

Cancel Anytime: If you cancel early, you’ll only pay for the visits used at $225/visit.
